- Driver’s license
- Insurance card(s)
- Referral (If your insurance company requires a referral, you will need to get the referral from your primary care physician before your appointment.)
- Current list of medications you are taking
- Copies of pertinent previous medical records, blood work, procedure reports, radiology testing, etc

What should I do if I need to cancel an appointment?
Please call the office during business hours. Please cancel at least 2-3 days ahead so that your appointment time can be given to another patient who is seeking care.
